Instructions for Collection
With the at-home testing service, your healthcare provider can simply order a collection kit to be sent to your home. maintain comfort and privacy if preferred while accessing the services of precision genomics.
Once you receive your collection kit bundle, follow the following simple 3 steps:
1. Fill in the provided form with your information
2. Collect a DNA sample with a simple cheek swab as demonstrated.
3. Send us your DNA sample
Once the sample is received, analysis of your DNA sample will take place, and the results will be sent back to your Provider within a few days.

Read and Sign the PGx Consent Form.
put the completed form in the provided Return Envelope

Provide your Insurance information in the provided requisition form.

Step 1C
Patient's Information
Fill your DNA sample slit Identifier
Input your name, date of birth, and the date you've done the DNA sample collection
Step 2
Collect Your DNA Sample
Open provided swab seal by pulling sides.
Insert the swab into your mouth.
Roll the swab about 5 to 10 times on the inside of your right cheek.
Repeat the same thing using the second swab for the inside of your left cheek.
Refrain from food or drinks at least 20 minutes prior to giving DNA sample.
Important
Carefully place the swab back into the collection pouch and seal it.
Step 3
Ship The Sample to Us
1. Signed patient consent form
2. Insurance information form (If sent by email, Ignore this part)
3. Your DNA sample pouch sealed with swabs inside
Place the following into the provided FedEx return envelope:

B. Ship with FedEx
Choose the easier option for you

1. Drop off envelope at a FedEx Drop Box or at a FedEx location. Search fedex.com/locate to find a location near you.

2. Schedule a pickup by calling 1.800.GoFedEx (1.800.463.3339). you don't have to pay them as you have a prepaid envelope.
